Claim Forms

Our claims service is vital in preserving the partnerships we have established. Therefore, we believe our interactive claims submission forms allow us to streamline your claims submissions. It is important to follow the claim form guide below to avoid incorrect submissions.

When initially downloading and opening the forms, they will open up in your default web browser (IE, Chrome, Firefox , Edge, etc.) Note: The forms will not display in your browser as they are made specifically for Adobe Reader. In order to edit the fields in the .PDF you will need Adobe Acrobat Reader which can be obtained free from the Adobe website.

STEP 01

When the document downloads, save the document to a location on your local hard drive.

STEP 02

Launch Adobe Acrobat Reader and from within the application open the file you have saved on your hard drive.

STEP 03

Fill in the necessary information in the interactive .PDF

STEP 04

Either digitally sign and save the document or Print it out and sign the document.

STEP 05

Email the document to the relevant department
